For a well at 417 McAndrew Road (State Well No. 04N22W04P04S), the Board voted to impose a one-time $500 fine and a $50/day civil penalty running from the June 19, 2026 compliance deadline. However, the daily penalty will be waived if the owner pays the outstanding balance of $3,357.50 by August 31, 2026. The owner had already made a partial payment of $5,025.
